INFORMED CONSENT STATEMENT
The IUS Happiness Survey

 

You are invited to participate in a research study.  The general purpose of this study is to assess the degree of happiness reported by students at Indiana University Southeast.  

INFORMATION
You are requested to complete the survey that begins on the next screen.  Survey questions will ask you about various aspects of your happiness.  It should take you approximately 25-30 minutes to complete the survey.  
As part of your participation in this survey, you will be requested to provide some basic demographic information such as your date of birth, gender, occupation, and ethnic identification.  You will also be asked to provide some academic information, such as your major, class standing, and your University Identification Number (UID).  Please note that your UID will be used for the purposes of this study only to access certain academic records, such as your GPA and progress towards the completion of your degree.  You will be asked about what types of activities you do to increase your happiness, such as help other people, participate in physical or social activities, and consume or take illegal drugs.  Please be advised that you may leave blank any of the items you do not wish to answer.  
It is estimated that approximately 500 students will be asked to participate in this survey during this semester.

BENEFITS
You will receive no direct benefits from participating in this research study.  You will, however, have the opportunity to examine your happiness and contribute to the body of knowledge attempting to assess the role happiness plays in the personal and academic success of college students.  

RISKS
There are no foreseeable risks involved in the participation of this study.  However, some of the information collected is of a sensitive nature (e.g., the use of illegal drugs), and, if known outside of the laboratory may put you at risk for civil or criminal liability. Even though we do our best to keep your data completely confidential and secure, we cannot prevent a court subpoena of any data (should the court know of your participation in this project and desire to see some of your data).  No reports of these studies will identify individuals.  
To help protect the security of the data you provide, all of the information gathered as part of this project will be kept on a secure computer storage file maintained by IUS that will be held separately from this web site. This procedure is designed to protect your responses from being viewed by unauthorized individuals not associated with the study and IUS.  Since a principal purpose of this study is to investigate the relationship between personal happiness and academic progress, identifiable student information will be maintained on the secure computer storage file for the duration of the student’s academic career at IUS.  Once that career is over, all identifiable student information will be removed.

CONFIDENTIALITY
Your survey responses will be sent to a link at the Institute for Learning and Teaching Excellence (ILTE) at Indiana University Southeast, c/o Ms. Katy Wigley, where data will be stored securely.  The ILTE will gather and report the information only to those individuals directly associated with the study.  Therefore, your responses will remain as confidential as possible.  However, when information is on the Internet, please be aware that there is always the possibility that it could be accessed by an unauthorized individual.  To help maintain confidentiality, please note that all of the information gathered as part of this study will be kept in a secure location maintained by the IUS Office of Information Technology, and your student identification number will be removed from the records once the academic information is no long necessary for the purposes of this study. Every precaution will be taken to maximize the security of the information.
Reports based on the results obtained from the survey responses will be written primarily in an aggregate form that combines the responses of many individuals.   However some individual responses will be used in the reports but without any individuals being identified by name or university identification number.

PARTICIPATION
Your participation in this study is voluntary; you may refuse to participate without penalty.  If you decide to participate, you may withdraw from the study at anytime without penalty and without loss of benefits to which you are otherwise entitled.  If you withdraw from the study before data collection is completed, your information will be removed from the database.
If you are being offered extra credit points for your participation in this study, please print a copy of this page now and present it to your instructor as evidence of your attempt to participate in this survey.
